I have a binder in my bookshelf where I keep all of the student's book recommendations that way when a student is wondering what they should read next they are able to grab this binder and read through the different responses.
Students have the opportunity to create a new cover for their book, come up with a "hook" to capture the readers attention, and even write their own blurb. (information on the back of the book) My kids love this because they are allowed complete creativity.
I have also used this sheet as an alternative to the traditional book report. It's wonderful because it requires deeper thinking and reflection about the text.
